新增了这层约束：
- 本地文件检索与 rag_retrieve / table_rag_retrieve 不是同一数据源
- 不能因为 RAG 查到了，就假设本地文件也被覆盖
- 也不能因为本地文件没查到，就推断 RAG 知识库里也没有
- 进入 fallback 时，仍需按顺序继续尝试本地文件检索

🐛 fix: 修复 GuidelineMiddleware 导致 assistant message prefill 报错
enable_thinking 开启时，thinking 中间件将 AIMessage 追加到 messages 末尾，
导致不支持 assistant prefill 的模型返回 400 错误。
修复方式：在 AIMessage 后追加多语言 HumanMessage，确保消息以 user 结尾。

🐛 fix: 修复 prompt 模板花括号转义及 tool_call_chunk 安全访问
- prompt 中 JSON 示例的 {} 被 str.format() 误解析为模板变量，转义为 {{}}
- tool_call_chunk 改用 .get() 安全访问，避免 KeyError
